About the Role
As a Hospital Community Outreach Counsellor, you will support children, youth, and young adults experiencing mental health, behavioural, substance use, or developmental challenges. You will work as part of the Child, Youth & Young Adult (CYYA) Mental Health & Substance Use Services Transition Team.
You will report clinically to the Clinical Practice Leader and administratively to the Manager or designate.
This role combines hospital support with community outreach services. You will help clients transition smoothly from hospital care to community-based programs.
Key Responsibilities
1. Client Assessment and Mental Health Support

Assess emotional, behavioural, and mental health status

Conduct mental status exams when required
-
Document accurate and timely clinical information
-
Monitor client progress and reactions to medications
You will use trauma-informed and evidence-based approaches when supporting children and families.
2. Individual and Group Counselling
You will provide:

One-to-one counselling sessions
-
Group therapy sessions
-
Psycho-education programs
-
Crisis intervention support
-
Behaviour management strategies
-
Conflict resolution and de-escalation techniques
You will also provide support and education to families and caregivers.
3. Case Management and Treatment Planning

Triage new referrals
-
Develop recovery plans and treatment goals
-
Participate in case conferences and team meetings
-
Support home visits and outreach services
-
Share workload with multidisciplinary team members
Your role ensures clients receive coordinated and consistent care.
4. Community Outreach and Advocacy
You will connect clients with important community services, such as:

Healthcare providers

Ministry programs
-
Educational services
-
Recreational programs
-
Other support agencies
You will help families navigate services and ensure smooth transitions from hospital to community care.
5. Documentation and Record Keeping

Maintain accurate client records
-
Use clinical information systems like PARIS

Prepare reports and progress notes
-
Follow privacy and Freedom of Information guidelines
Proper documentation ensures quality care and regulatory compliance.
6. Medication and Daily Living Support

Assist clients with medication management
-
Support daily living activities
-
Escort clients to appointments and community programs
-
Drive clients when necessary (valid license required)
You may also supervise social and recreational activities.
7. Team Collaboration and Professional Development

Participate in team planning and meetings
-
Support volunteers and new staff orientation

Assist students during training placements
-
Contribute to program development and quality improvement

Stay updated on mental health legislation and best practices
You will actively contribute to service improvement initiatives.
Qualifications and Requirements

Education

Bachelor’s Degree in Child and Youth Care, Social Work, or a related health discipline
Experience

Minimum one (1) year recent experience working with children and youth with mental health issues

Experience in a multidisciplinary healthcare setting
License

Valid BC Driver’s License

Access to a personal vehicle with business insurance
